Create a new page Pages –» Add Page
Select 'Contact Form' as its Page Template from Page Attributes box.

Scroll at bottom and where you see 4 layout icons in the section of 'Theme Options Custom Settings' and choose the one which is a full blank box (i.e. 2nd from left).

Go to Theme Options –» Contact Page –» Contact Information.
Click the checkbox 'on' for "Enable the contact informal panel" and fill in your required details in the input boxes below and save changes.

[Optional step]: If you want to show an image in top part, go to Add Media –» upload a image (preferably horizontal). See in demo


How to show Google Maps:
Go to Theme Options –» Contact Page –» Maps, and put in the Google Maps Coordinates.
Additionally, fill in the input fields below (if required).

Note: Please read the description there to understand how to get the co-ordinates.


How to display Twitter widget:
Go to Theme Options –» JS Twitter Widget.
Here you need to fill in your Consumer key and Consumer secret before you can authorize a Twitter account. The directions to get the Consumer Key and Consumer Secret are mentioned there. Once you have done the steps successfully, only then Twitter feed will automatically be visible on the Contact Us page.
